Some more investigation reveals that the additional vertex seems to be the midpoint between two other vertices, and should've been removed by simplify.

The difference ended up being which geos library it'd been built with. I've seen this many times especially with GEOS/Shapely. Few years ago we had an issue where one lake was disappearing on a map due some rounding error. | 09:49:01 |
